Knee pain. Can anyone help "diagnose"?

Before I'm asked, yes, I have seen an orthopedic/sports dr, have had an x-ray, and done phys ther. However, I never got a clear cut diagnosis, so maybe someone out there has had similar symptoms/problems?

In my left knee:
-Pain in the "soft spot" between the knee cap and tibia
-After a few months of PT and the assigned stretches, the pain goes away, and I can stop doing the stretches, so long as I keep active (playing tennis regularly). However, in about mid-winter, the pain starts again when I go from being inactive (Nov-Jan) and start skiing and conditioning for tennis.
-Hurts doing lunges (when starting to stand back up), when doing stairs, holding my leg out and straightening it, and sometimes when running.
-During PT, when my therapist felt for swelling, there was also severe pain with pressure on the middle back of the knee, but it doesn't seem to be there all the time.

Sound familiar to anyone?

1. sure sounds familiar to me I am sitting here re habing at the same time writing to you,, on the 12th of Dec I had my left knee re replaced becaise the old one wore out.. I had my right knee replaced in 2002' Here is what you do... go back to the Ortho 'doc and as him if you are eligable for a Synvisc series of shots. My series made my knee feel good for two years and when we tried again it did not help for long, maybe a month.. then we had an MRI (which you should have ASAP) I was a rodeo clown forf 17 years and my knees took a heck of a beaten.. the knee I just had re replaced gave out on me and I have the parts they took out...
