2. one common case can be blood, that are more than enough blood in the brain area, so it pushes to the skull hurting the core. But there are lots of other reasons

a reason is when you think a lot (using a lot a videogame, using alot a pc, reading a lot)

4. The pain has multiple causes but changes in the vasculature is one reason (the blood vessels contract or expand) (ie ice-cream headache). Also a pressure differential in the sinuses can lead to pain (sinus headache). The pain is a nerve reflex, so it's really the sensory neurons surrounding the brain that causes the headache.
